Interstitial lung disease (ILD) describes a complex group of over 200 disorders that cause progressive inflammation and scarring (fibrosis) of the lung tissue. While rare in children, these conditions require careful, specialist assessment and long-term management. Understanding ILD in children
Childhood ILD (sometimes referred to as children's interstitial lung disease, or chILD) encompasses a wide range of conditions affecting the lung tissue itself, rather than just the airways. Causes vary widely and can include genetic conditions, developmental lung abnormalities, autoimmune conditions, and, in some cases, no clearly identifiable cause.
Signs that may prompt investigation
Persistent rapid or effortful breathing
Reduced growth or slow weight gain
Reduced tolerance for exercise or activity
Chronic cough not explained by more common causes
Low oxygen levels identified on examination or testing
Recurring or unexplained respiratory symptoms in infancy
Diagnosis & Management
Diagnosing ILD in children is often complex and involves detailed imaging (such as high-resolution CT scanning), lung function testing where age-appropriate, and in some cases, genetic testing or further specialist investigation. Dr Kok works closely with paediatric respiratory teams and, where needed, tertiary specialist centres to reach an accurate diagnosis.
Because ILD encompasses such a wide range of underlying conditions, management is highly individualised and may involve:
Regular monitoring of lung function and growth
Coordination with multidisciplinary and, where relevant, tertiary specialist teams
Supportive therapies to manage symptoms and support daily function
Ongoing family education and support, given the long-term nature of these conditions
